GetWellNetwork’s Outcomes Consulting Team, comprised of clinicians and former health system leaders, supports our client partners in integrating IPC into clinical practice and operations. We combine our experience with a rich set of tools and a collaborative, 90-day process to design clinical workflows that fit your organization’s vision for IPC.
Successfully Integrating IPC into Clinical Practice
The Clinical Practice Design engagement has proven to be highly effective in gaining staff adoption, stronger utilization and improved outcomes. Using a shared governance model, the CPD process actively engages front line staff to design and build their own workflows in areas such as patient education, pain management, medication teaching and discharge preparation.
The process begins by engaging your organization’s senior leadership to develop a vision for IPC, establish outcome priorities through an Outcomes Achievement Plan (OAP) and select four or five targeted outcomes to improve through redesigned care workflows. With the IPC vision and OAP as context, hospital staff evaluates current practice, identifying what works and what needs to be changed for greater quality and consistency in the care process. Teams around each of the selected workflows then redesign these core processes to align with the organization’s vision and advance care quality, consistency and outcomes.
Once designs are complete, our Outcomes Consultant serves as your liaison with GetWellNetwork’s Pathway configuration team to build the tools to support these newly designed workflows. We also work with your senior leadership to establish a plan for change management and cultural adoption specific to your organization. Once final approval is provided by your executive team, an Integration Consultant partners with your departmental leaders to support implementation of the new workflows and the integration of IPC into daily practice.
